Please be advised that the Virginia Youth Soccer Association and the Fredericksburg Cup Soccer Tournament does not require the medical release forms to be notarized.  If your team already has current, up to date, medical release forms on file and they are approved by your state or national association/federation then you can use them for the Fredericksburg Cup in place of this one.  Please click here for the medical release form.
